He probably died from claustrophobia.
He was a Beta fish, right? I've heard they're pretty hard to keep. So don't blame yourself too much.
That is pretty sad. It's good that Addy hadn't gotten attatched or you could've had a major problem with her reaction to his death. Believe me, it's not fun helping your kids deal with the death of a pet.
